If you would like to become a patient of Dr. Wise, please follow the steps below:

To make an appointment please telephone 020 7631 1433 during working hours or leave a message on the answer machine and our Practice Manager will contact you. If calling from outside of the UK dial: +44 20 7631 1433.

If you have been referred by your own dentist or doctor it is helpful if you can arrange for a referral letter to be sent to us.

We will send you a contact letter and a medical history form with a stamped addressed envelope. Could you please ensure that the medical history reaches the practice prior to your appointment.

An initial examination will be necessary. Please see the Fees pages for more details.
